Yesterday it was reported as likely as this morning it was confirmed. Derrick Goold of the St. Louis Post-Dispatch solidified the speculation via this Tweet Wednesday morning:
Alex Reyes, after review, will have a Tommy John surgery, #cardinals affirm. Paletta will do surgery here in Jupiter.

Reyes, who is still considered a prospect, performed at an elite level last season making 12 appearances, 5 of them starts. He didn’t give up more than 3 earned runs in any game he started and was in line to start the 2017 season in the rotation. Before the injury he was considered the best pitching prospect in baseball.
With Reyes out for the year, Michael Wacha now has a wide open lane to secure a job in the starting rotation. Still only 25, Wacha impressed in 2015 by collecting over 180 innings pitched and keeping his ERA at a respectable 3.38. However, last season his dismal 5.09 ERA resulted in him losing his starting gig.
